HOW TO RESERVE YOUR SPACE

There are two methods you can use to reserve a spot for your child in YMCA Afterschool programs.

By fax or mail

  1. Print out and complete Youth Program Registration Form below.
  2. Print out and complete immunization form.
  3. You can mail or fax to your local YMCA branch - see below or bring the forms with your to your YMCA. We will contact you to complete the process and discuss payment plans.

Or visit your local YMCA branch.

  1. Request and complete reservation forms at your local branch.

 

It is highly recommended that you return completed registration forms at least two weeks prior to expected start date. Once your reservation has been confirmed, you will be mailed or given additional forms to complete your child's registration. Most programs are limited in size. The YMCA reserves the right to change, combine or reschedule programs. In the event of a cancellation, a full refund will be issued.
